Project
Summary:
NORCAS, Great Yarmouth, has identified the need to provide a domiciliary-based, Community Support Worker for people with drug/alcohol misuse problems. The aim is to improve an individual?s chances of maintaining independent living, reduce constant re-offending and reduce marginalisation.
The Community Support Worker?s role is to support people in their own homes, enable individuals to maximise their capacity to live in the community, provide a link between the client and relevant agencies and co-ordinate care action plans.
